New Patient Information

What to expect your first time here:

Expect to be at our office anywhere from 1-2 hours for your first visit. This time includes filling out the new patient forms (basic info, medical history, etc), creating you as a new patient in our system, possibly dilating your eyes, and the exam with our doctors.

**Please note that Dr. Yalon is currently the Chief of Ophthalmology with the Memorial Healthcare Network and is one of the doctors on-call with the Emergency Rooms. We adjust and plan our availability for regular appointments according to his on-call schedule. We do our best to keep you informed if there are any extensive or out-of-the-ordinary delays...but we do prioritize emergencies. We believe everyone's time is valuable and do our best to respect it! You will have the option to wait and keep your appointment or reschedule if necessary.

  • Insurance Card (Primary AND Secondary Insurance) You won't need to bring it every time, unless your insurance company issues you a new one. **Due to the high level of fraud and our desire to keep your information protected we will only be able to see new patients who bring their insurance cards with them). 

 

  • Photo ID

  • Medications you currently take. If you have a list of current medications, please make sure it includes the dosage and how many times per day it is taken so we can record your accurate medication dosage. (If you do not have a list you can also bring the actual medicine bottles).

  • Eye Glasses you wear... even over-the-counter readers.

​

  • Referral/Authorization if you have an HMO plan.

What to Bring:

Hospital Referrals

 

**If you were referred by a hospital, please bring your discharge paperwork with you. *Please call our office to verify if we accept your insurance, hospitals do not necessarily refer you to a doctor that is in your network. If we do not accept your insurance... don't worry, you still have options:

  1. Call your insurance and request they refer you to an Ophthalmologist in your network.

​​

    2. Come to our clinic as a self-paying patient (will not be charged to your insurance company). This can be charged to your Visa, MC, Debit Card or paid as Cash. Alternative options are CareCredit or financing through Wells Fargo.

Going Eastbound on Hallandale Beach Blvd from US-1: The last traffic light before the bridge leading to the A1A is called Three Islands Blvd. (you will see a PNC bank on your right). The turn into our complex is directly after the traffic light on your right. We are located on the ground floor as a store-front (between the Nail Salon and the Dentist)... not inside the building.

Going Westbound on Hallandale Beach Blvd from A1A... just as your cross the bridge, the first traffic light is Three Islands Blvd. Make a U-turn and an immediate right turn into the complex (NOT the PNC Bank parking lot). We are located on the ground floor as a store-front (between the Nail Salon and the Dentist)... not inside the building.

You can download, fill out and print the forms and bring them to your appointment with you (this will shorten the wait time at your appointment). We are not currently set up to receive your forms online or via email due to HIPAA regulations...we're working on this feature!

Insurance Plans Currently Accepted:

If you don’t see your insurance plan listed, please call our office to verify if we accept your insurance. Our Ophthalmologists offer financing as well for those without insurance.

Please note, we currently do not accept vision plans, only health insurance plans.
